Colorectal diagnostic assessment program

What is a colorectal diagnostic assessment program (Colorectal DAP)?

 
The Colorectal DAP consists of two regional programs:

Regional Colonoscopy Network

The RCN is designed to decrease wait times for patients who are 50 to 74 years old with a positive fecal occult blood test (FOBT) or who have a family history of colorectal cancer. The RCN closely follows the colorectal screening guidelines from Cancer Care Ontario and Ontario’s ColonCancerCheck program.


How can I refer?
You can refer your patients with a positive FOBT (50 to 74 years of age) or where there is a family history of colorectal cancer.

 

The centralized referral office at the RCN screens and prepares patients for colonoscopies through a telephone consultation and books patients into the next available RCN screening appointment at one of four participating regional hospitals. These are Grand River (Kitchener), Guelph General, Louise Marshall (Mount Forest) and St. Mary’s General Hospital (Kitchener).

 

Over 15 regional gastroenterologists and surgeons participate in this program providing timely access to screening colonoscopy.

Colorectal nurse navigator

The Colorectal nurse navigator provides support and guidance to patients during the early phase of a new colorectal cancer diagnosis.


Patients are referred to the nurse navigator by regional surgeons and gastroenterologists post colonoscopy and biopsy.


The Colorectal nurse navigator provides support, education and guidance to patients during the early days of a new cancer diagnosis and acts as a patient advocate to obtain the earliest date for pre-operative testing and treatment. Support can be provided over the phone, or one on one in person. The Colorectal nurse navigator follows guidelines from Cancer Care Ontario regarding the diagnosis and treatment of colorectal cancer.
