Palliative care resources

Tools used within the palliative care team:

Cancer Care Ontario (CCO) symptom management guidelines

The symptom management guidelines developed by Cancer Care Ontario are evidence-based guidelines that help health care professionals in assessing and managing their patient’s symptoms.

CCO collaborative care plans

Collaborative care plans are guides to practice that focus on the stage the patient is at in their disease and their expected outcomes. They promote communication and continuity among team members.

Edmonton symptom assessment scale (ESAS)

The ESAS is a validated, subjective assessment tool that the patient completes, which is used to help assess nine common symptoms experienced by palliative care patients using a scale of 0-10.

Palliative performance scale (PPS)

The PPS is a valid tool that measures a patient’s functional status. It provides a framework for measuring a patient’s functional decline and is measured in 10% increments from 100% being fully ambulatory and functional, and 0% being death.
